If you would like to make a referral to the EAP of one of your employees, please review the article for managers entitled ""The Corrective Interview: Managing Job Performance & Personal Problems."
- For Formal Referrals, you will need to complete the EAP Supervisory Referral form and fax the completed form back to COPE (202-628-5111) once you have reviewed the form with the employee. We also strongly request that you contact COPE at 202-628-5240 to discuss the referral with an EAP Specialist prior to making the referral.
- For Informal or Suggested Referrals where you will not need the employee to sign authorization allowing the EAP to confirm participation, you do not need to complete this form. Simply give the employee the EAP number (202-628-5240) and suggest they call us.
